This is the syllabus for a Health Care Coordination course.
The goal of health care coordination is to improve patient outcomes with better health care services. Care coordinators play a critical role in improving patient care. Students will learn how to effectively advocate for patients and interact with members of the healthcare team in finding solutions to provide high quality, value-based, and efficient care. Effective communication styles, assessing patient’s needs and goals, and helping with patients’ transitions of care are among many topics covered in this course. Upon course completion, students will have acquired basic knowledge and skills to educate, engage, and support patients and their health care teams to improve health outcomes and quality of life.

A key theme of this course is that most health outcomes are driven by personal behavior choices, but that those choices are made in the context of the neighborhoods where we live, work, and play.  A corollary to this theme is that engaged citizens are healthier citizens (defined as a "resident of a particular city"). The object of this project is to take the principles learned in the classroom and apply them in a community environment: observe an issue in the community, document it through video/audio commentary, then use the tools of advocacy to address the issue.While designed for a Public Health course, this activity can be adapted for any topic where community organizing can lead to positive change.

In this writing assignment, students will conduct research on statistics about COVID-19 in New York City and their embedded communities using free online databases from the New York City Department of Health (NYC DOH). For questions 1, 2, 3, and 4, students will conduct research on COVID-19 statistics within NYC and their specific neighborhood/community in New York City. In questions 5 and 6, students will analyze the data you have gathered while using your own knowledge of your community. A full set of instructions for accessing NYC DOH databases and a complete rubric for grading the assignment is included.
